Middle of the year changes cheyyakudadu anedi Govt law kaadu. Its the rule by either your employer or the insurance company providing this service. In our small company, we make rules and change rules. I do have situations where we have changed our employees coverage in and out by talking to our insurance company. (But its hard since group coverage comes with a lot of restrictions).

if you are already an FTE , you cannot join into group health ins. in the middle of the year

if you are joining a new company as FTE and they offer health ins. they will give the option to join when you start employment - your emp start date can be any time of the year

if you are already FTE plus health ins. - typically you can add others to your health ins. for life events - pelli, pillalu etc
