Place the subtitle in the same folder as movie and file name should be same for both.

Ex: C:/Movie1.mp4, c:/Movie1.srt

Use handbrake http://www.videohelp.com/tools/HandBrake
or
Avidemux: http://www.makeuseof.com/tag/embed-subtitles-mp4-flv-videos- avidemux-crossplatform/

Downloaded subtitles (.srt) play avvatledha. I never had any issue like this.
