recently i submitted .. I ddint notarize any xerox.. Address proof original submit chesaa.. Passport data pages xerox(naadi wife di ) submit chesa.... Grand pa passport avasaram ledu.. H4 approval document printout. 6. needi and wife di Drivers license color printout front and back that shows your address(residence proof) 7. electricity bill and water bill original mails that you get to your home address. 8. needi and wife di passport all pages copies. 9. papa passport 1st page and last pages(last page numbers travisa site lo mention chesinattu thesuko) color printouts. 10. papa original passport. 11. papa birth certifcate copy 12. fedex return envolope and mention your home address 13. Money Order or Cashier's Check (Payable to Travisa Outsourcing) 12$ + 188$ 14.
