approved means USCIS approved? If so, you are WRONG. You dont need to get these done by Approved USCIS doc in India, If these tests are done(no doctor involvement here, its labs for tests), get those records and submit to the approved doc here in USCIS. This doc validates them for the eligibility and exempt those which are acceptable and get the missing tests only done here.. | ||

1) Xray is required if skin test fails. 2) you cant use finger xray for your tongue :-) its not one for all. Doc should fix it as per the need. :-) | ||

I guess I should be having that letter somewhere, I gotta dig for it from some pile of papers. In general, it should be a simple one liner (As per our records, the following patients has completed the listed tests .....) then patient full name (as per the passport) and then list of tests and dates next to each test. With Doctor sign, rubber stamp and registration number etc.. on the hospital letter head. (its just for the doctor office in US, it wont be sent to the USCIS). Here you go(from my records for a GC application for parents over a decade ago, check for the latest) Get Medical Examination by the INS approved doctor. Medical Tests: Vaccination (If done in India, get a letter from India) · H1N1 Flu vaccination · MMR (mums, measles, rubella) · TD (Tetanus and Diphtheria Toxoids) · Varicella (aka Chickenpox) Local doctor does these (if you can get a letter from India that these are done within the last one year, you can avoid these also) · Skin Test for TB(or XRay) · Blood Test: HIV · Blood Test: Syphilis | ||
