aa mandated amount enta low anedi naa koschen. I don't think govt pays the inflated rates of doctors. They pay standard medicare rates and many doctors won't accept it. This is a chicken and egg problem of course

ila avadhu. Try finding real life scenario..
You may not get Doctor you prefer but you will get Doctor. The rates are mandated. And that I think is one of the reasons insurance companies are bailing out.
Medicare lo hospitals already ilanti hits teesukuntayi!! They don't have choice there.. but ikkada the risk is on the insurance provider.

mandatory ga pregnancy cover cheyyali ani undhi anukunna....

I did not understand this specific scenario but in general pregnancy/ maternity case is the one of the 10 essential health benefits that must be included.

This was mandated before ACA also but only in group insurance policies that have more than 20 employees. Individual plans were mandated by states.(Ny and Ca mandated coverage ). So most states let insurance companies remove coverage. ( when state mandates, these are somethings to consider)

as per ACA , pregnancy / maternity care is included as mandatory in individual plans and that was the most impactful area of ACA.

Group plan lo irrespective of ACA , they should not reject. I am really surprised on the fine print you ran into!! And that really sucks .. no pre-existing means no pre-existing.. particularly in case of health essentials they listed as per ACA.

But under the health reform legislation that President Obama signed in 2010, all individual and small group plans purchased on or after January 1, 2014 (including plans sold through the exchanges and outside the exchanges) must include coverage for these ten essential health benefits with no annual or lifetime dollar limit:

hospitalization
ambulatory services (visits to doctors and other healthcare professionals and outpatient hospital care)
emergency services
maternity and newborn care
services for those suffering from mental health disorders and problems with substance abuse
prescription drug;
lab tests
chronic disease management, “well” services and preventive services recommended by the U.S. Preventive Services Task Force (including blood pressure screening, breast cancer screening, colorectal cancer screening, obesity screening and counseling; tobacco use counseling and interventions, and breast-feeding counseling)
pediatric services for children, including dental and vision care (there is some flexibility on the inclusion of pediatric dental if the plan is purchased within the exchange)
rehabilitative and “habilitative” services which include helping a person keep, learn or improve functioning for daily living. (Examples include therapy for a child who isn’t walking or talking at the expected age physical and occupational therapy, help for those experiencing problems with speech, and treatment for individuals suffering from a variety of disabilities.)

There are still grandmothered and grandfathered plans in force that do not have to include all of the essential health benefits. But every individual and small-group policy sold since January 1, 2014 includes essential health benefit coverage.

I was in the same boat back in 2014, took a new insurance policy when my wife was 7 months pregnant.. all claims were approved.. that was the first year of Obamacare...

Stellar:

pregnancy ki riders enti.. kothaga vintanna..

https://www.healthinsurance.org/obamacare/how-obamacare-chan ged-maternity-coverage/

pregnancy was considered a pre-existing condition prior to obamacare with some exceptions in some plans...

Even before the ACA, maternity coverage was available on most group plans, thanks to the 1978 Pregnancy Discrimination Act. The Act mandates that if an employer with 15 of more employees opts to provide health insurance, the coverage must include maternity benefits. And in nearly forty percent of the states, there were regulations that required small group plans to include maternity benefits, even if the employer had fewer than 15 employees.

what are the alternative plans if the existing coverage is not covering a particular treatment?

Insurers can't deny coverage for pre-existing conditions. Do you mean an experimental treatment? Insurance doesn't cover experimental treatments.
